He was in pole position after getting ahead of Tyler Farrar and Alessandro Petacchi as the finish loomed.
But Belarussian Yauheni Hutarovich shot past Cav on the outside to edge him into second.
The Manx Missile does however keep the leader's red jersey for another day.
Meanwhile Cav's fellow Manxman Peter Kennaugh finished in 69th, one minute behind the leaders.
Tomorrow's 157km third stage takes the riders from Marbella to Malaga.
